REGISTRATION
INFORMATION
Registration for Individuals: $350.00 per person on or before November 6th; $400 per person thereafter. Includes
attendance to the 2009 Symposium, a full-day conference, including breakfast,
luncheon & reception, and a copy of the Forum's Leadership and Referral Guide.

Corporate Table Purchase:
Standard Table Purchase: $3,500 on or before November 6th; $4,000 thereafter. Includes full attendance to the 2009 Symposium (see above) and copies of the Forum's Leadership and Referral Guide for 10 individuals. Also includes recognition for your organization in the Symposium Program and on signage at your luncheon table.
Standard Half-Table Purchase: $1,750 on or before November 6th; $2,000 thereafter. Includes attendance for 5 individuals, with appropriate recognition.
Premium Table Purchase: $4,000, on or before November 6th; $5,000 thereafter. Includes all of the benefits of Standard Table Purchase, plus recognition as Gold Partner in Symposium Program, Leadership & Referral Guide, on Forum website and on Forum signage, and an invitation for (1) to Invitation-Only 2009-2010 “Conversation” events.
The Symposium program may qualify for up to 5.5 hours MCLE and CPE credit.
