Registration for Individuals:

$350.00 per person for Symposium in Advance; $400 per person Full Price. Includes attendance to the 2017 Symposium, a full-day conference, including breakfast, luncheon & reception, and a copy of the Forum's Leadership & Referral Guide. See registration form.

Corporate Table Purchase for Symposium:

Basic Table Purchase: $3,500 in Advance; $4,000 Full Price. Includes full attendance to the 2017 Symposium (see above) and copies of the Forum's Leadership & Referral Guide for 10 individuals. Also includes recognition for your organization in the Symposium Program and on signage at your luncheon table.

Basic Half-Table Purchase: $1,750 in Advance; $2,000 Full Price. Includes attendance for 5 individuals, with appropriate recognition.

Preferred Table Purchase: $4,000, in Advance; $5,000 Full Price. Includes all of the benefits of Basic Table Purchase, PLUS recognition as Gold Partner in Symposium Program, Leadership & Referral Guide, on Forum website and on Forum signage, and invitation(s) for one year to the “invitation-only” Conversation events and the “invitation-only” Leadership Connection event.

The Symposium program may qualify for up to 5.5 hours MCLE and CPE credit. Contact us for details.
